Patients may be seen at Anacortes Physical Therapy with or without a written referral for physical therapy. Many insurance companies, however, require a written referral and/or pre-authorization for payment of physical therapy services. We accept most insurance, including, but not limited to Premera Blue Cross, Regence Blue Shield, Medicare, Labor and Industries claims, DSHS, and motor vehicle PIP coverage. Please be prepared to give our receptionist your insurance information. We will make every effort to verify your benefits before treatment begins. If you have both a primary and a secondary insurance, we will bill both for you.
How to schedule an appointment:
Anacortes Physical Therapy does our best to accommodate same-day appointments if needed. In most cases, we will be able to schedule your initial visit within 24 hours of your call. We accept most insurance plans, and private-pay arrangements are also available.
Before your first visit:
Please download and complete our patient registration and consent forms and then bring them to your first visit to our clinic. You may also fax the form to us at (360) 299-3038.

What to expect at your first visit:
On your first visit, you should arrive 15 minutes before your scheduled appointment time to complete the patient registration and consent forms, if you have not already done so.
Please bring your insurance card, your prescription for physical therapy, and any notes or reports from your physician that describes your condition. We also encourage you to bring any x-rays or other test results that your physician may have given you.
Wear loose-fitting, comfortable clothes to allow access to the affected area (e.g. shorts, t-shirt, sports bra, etc.).
Plan for the session to last one hour. Your past medical and current injury history will be taken and a physical examination will be performed to determine the nature and severity of the injury or symptoms. The exam will be followed by education on the current diagnosis, prognosis and a home exercise program.

Billing and Insurance information:
We will be happy to bill your insurance when you present your insurance card, a claim number, or other pertinent information. We attempt to verify coverage with your insurance company, including yearly dollar coverage limits, the number of approved treatment visits for therapy and the need for pre-authorization of physical therapy treatment sessions.
Co-payments for each visit should be paid during the appointment as well as any deductible requirements. We will file any necessary insurance claims and let you know if any additional payment is required. We accept payment by cash, check or credit card.
Anacortes Physical Therapy is committed to providing our patients with kind consideration in response to financial situations beyond your control. There are various options of repayment that can be discussed and we encourage dialogue with our accounts manager regarding these options.

We also accept coverage for third party motor vehicle accident and personal injury protection claims that involve medical and legal intervention. Anacortes Physical Therapy is committed to provide an account balance up to $3000.00 before requesting subrogation or requiring some personal payment on your account. This service of providing a medical loan, in essence, is a gesture of kindness to allow injured individuals who do not have access to personal compensation or reimbursement but are in need of physical therapy care.
